Types of Closed Rhinoplasty

The two types of Scarless Rhinoplasty performed at Scarless Nose Institute of Beverly Hills are the Closed Scarless Rhinoplasty (Scarless) and the Scarless Functional Rhinoplasty. Dr. Dugar takes a specific and custom approach to each patient depending on the type or the reason for the procedure. Below are the various reasons that a person may need to receive a scarless rhinoplasty as well as examples of how each demographic may be treated with a different type of scarless rhinoplasty.

Boxy or Round Tip

A Boxy, Round or Bulbous tip can be a major concern because it doesn’t allow the elegance of your nose to be shown. A refined nasal tip can make a subtle but extremely meaningful difference when done correctly.  We perform refinement of the nasal tip through a scarless approach using endonasal techniques to size, shape and positioning of the nasal tip cartilages. Depending on your need we have an answer to address it with our closed scarless technique. Sometimes it's as simple as carving and sculpting your current cartilage.  Sometimes we need to reposition the tip cartilages with delivery and suture techniques.  Sometimes we need to use small grafts to enhance the projection or rotation of the tip. Whatever is required, it will be done scarless and look natural, to keep your scarless secret.

Wide Nasal Bones

Wide nasal bones can cause the nose to look wide and sometimes flat. These wide bones can make the nose difficult to contour when wearing makeup.  A common trick makeup artists use to camouflage a wide nose is “nose contouring,” where straight lines are drawn down the nose to make it appear thinner. With our closed scarless approach we can permanently “contour” your nose by narrowing the nasal bones to give a more elegant refined look to your nose from all angles. Whatever is required, it will be done scarless and look natural, to keep your scarless secret.

Dorsal Hump (Bump on the Profile)

A Dorsal Hump (or Bump on the profile of the nose) is one of the most classic reasons a patient seeks out Closed Scarless Rhinoplasty. The elegance a nose can have from the removal of a dorsal hump is incredible.  Michelangelo once said, "Every block of stone has a statue inside of it, and it is the task of the sculptor to discover it.” This has never been more true than with the removal of a dorsal hump. This is one of the most challenging parts to do outstanding in a scarless rhinoplasty. Most surgeons gloss over this step considering it to be “easy.” We think the exact opposite. We truly believe getting an outstanding profile is one of the most difficult steps in a scarless rhinoplasty and we spend enormous time with every nose perfecting the profile. Dr. Dugar will remove bone and cartilage in hundreds of micro-millimeter slivers to slowly and carefully sculpt the profile to the perfect balance. In men, we like to keep a dominant profile, sometimes even with a high point, that stays masculine with no telltale sign of surgery. In women, we like to feminize the profile but preserve a strong profile that doesn’t cause a “scooped” or “ski-slope” appearance. Whatever is required, it will be done scarless and look natural, to keep your scarless secret.

Asymmetric Nose

Asymmetric noses can plague photographs and selfies. We commonly get asked about fixing asymmetric features of a nose. When they are dramatically asymmetric, we have techniques that can address this extremely well. But when it is very subtle, we like to remind everyone that no one has a truly symmetric face or nose. If you take the 10 most beautiful people and compare their computerized symmetric face vs their natural face, the natural face always looks better. This is because what is more important than symmetry, in a face, is contours. Contours are what draw the light to and from the face and can make or break how the light reflection enhances or shadows features. When there are significant contour irregularities causing asymmetries of the nose, we can fix these using our scarless techniques. Whatever is required, it will be done scarless and look natural, to keep your scarless secret.

Droopy Tip (Hook Nose)

Droopy tip (or Hook nose) is one of the most common reasons patient seek out Closed Scarless Rhinoplasty. This can be a concern when at rest or even worse when smiling. This can be seen almost everyday and it can really bother some people. The good news is that is there is a relatively simple and extremely safe approach to correcting this with our scarless technique.  The key is to rotate the nose upwards and prevent the excess pulldown when smiling. But care must be taken to not rotate it too much and cause a “Miss Piggy” effect. We utilize multiple maneuvers to carefully rotate the tip of the nose to lift it to an elegant height and design without any telltale signs of surgery. Whatever is required, it will be done scarless and look natural, to keep your scarless secret.

Crooked Nose

A Crooked Nose can plague photographs and selfies. Fixing a crooked nose can be challenging, even more the most talented rhinoplasty surgeons. But with specialized techniques we are able to straighten and improve a crooked nose significantly. We utilize a combination of dynamic grafting, septal deviation correcting, upper lateral cartilage correction, and nasal bone repositioning to achieve straightening of the nose. While there is no such thing as a perfectly scientifically straight nose in real life, getting your nose “straighter” is the goal to achieve a contoured look that blends in naturally with the rest of your face. Whatever is required, it will be done scarless and look natural, to keep your scarless secret.

Female Scarless Rhinoplasty

Female Scarless Rhinoplasty is one of our most common. The anatomy of a beautiful female nose cannot be taught in a textbook.  It is taught in life, on the streets, in magazines, on TV and in movies. Studying beautiful faces allows Dr. Dugar to understand the look and feel of a beautiful female nose.  The contour of a female nose is so important to be able to visualize before surgery in order to deliver the best results. When it comes to the profile of a nose, there is always a question of whether to make it perfectly straight, whether to take away a couple of more slivers to allow a small amount of a scoop, or paradoxically to allow a few extra slivers to stay and actually preserve a tiny high spot. All of these can look good depending on the specific face, and this is where Dr. Dugar’s expertise and knowledge and your opinion and preference all meet for the perfect delicate balance.  As for the tip of the nose and the bones, making an elegantly contoured nasal tip and nasal bones is a passion for Dr. Dugar. Breathing surgery is often a component with female scarless rhinoplasty, where sometimes we perform a Septoplasty (breathing surgery to fix a deviated septum) alone vs a combination Septorhinoplasty (breathing surgery to fix a deviated septum in addition to cosmetic rhinoplasty). Whatever is required, it will be done scarless and look natural, to keep your scarless secret.

Male Scarless Rhinoplasty

Male Scarless Rhinoplasty is very special. Dr. Dugar has tremendous experience performing male scarless rhinoplasty which is extremely important as it is almost a completely different surgery than female scarless rhinoplasty. It is of utmost importance that a male nose must stay masculine.  The masculinity of a male nose comes from preserving certain features including preserving a high spot on the profile, not turning up the tip too much, not sculpting the tip too much, not making the bones too narrow. It is essentially one of the most conservative surgeries we perform, yet most surgeons treat a male nose just like they would a female nose. Preserving the masculine architecture while delivering meaningful refinement is the key to success for male scarless rhinoplasty. Breathing surgery is often a component with male scarless rhinoplasty, where sometimes we perform a Septoplasty (breathing surgery to fix a deviated septum) alone vs a combination Septorhinoplasty (breathing surgery to fix a deviated septum in addition to cosmetic rhinoplasty). Whatever is required, it will be done scarless and look natural, to keep your scarless secret.

Teenage Scarless Rhinoplasty

Teenage Scarless Rhinoplasty is a sensitive issue and topic and one that involves the family closely. There are two extremely critical factors when considering to operate on a teenager: Nasal maturity and Emotional maturity. Nasal maturity is when the nose has essentially stopped growing and the cartilage and bones are mature enough to withstand the trauma of a rhinoplasty. Usually nasal maturity is achieved by age 15+ for most noses. There are sometimes 17-18 year old noses that are still not mature physically, so it is anatomically specific for each patient individually.  Dr. Dugar will assess your or your teenager’s nose to identify its nasal maturity. Emotional maturity is even more important. Dr. Dugar reads people fast and is interviewing you the entire time to decide if you or your teenager is mentally and emotional mature enough to understand the consequences of a scarless rhinoplasty. For some teenagers, undergoing a scarless rhinoplasty can be positively life changing – it can end bullying or teasing from their peers, can increase their self-confidence and increase their awareness and motivation in life. Countless times, Dr. Dugar has seen young teenagers, that he has performed a Scarless Rhinoplasty on, grow incredible confidence post-operatively due to the insecurities of their nose now gone.  Sometimes a nasal deformity can really bring on a significant amount of stress and tension on a person, young or older, and it's important not to demean it or ignore it. A conversation about it is important. One of the most powerful things Dr. Dugar loves to do is ironically empower young teenagers who he truly believes don’t need surgery.  He boosts their confidence and sometimes hearing it from a Beverly Hills Celebrity Plastic Surgeon is all they need for a kickstart to positive mentality of their beauty. Everyone is truly beautiful and no one needs a scarless rhinoplasty…but if it can help remove insecurity and instill confidence, do it. But do it Scarless…  Whatever is required, it will be done scarless and look natural, to keep your scarless secret.

Asian Scarless Rhinoplasty

Asian Scarless Rhinoplasty is extremely special and requires careful skill and knowledge of the nasal anatomy. For most Asian noses, the problem lies in weak cartilage and lack of bony or cartilage projection of the profile. This leads to a weak dorsum that can appear flat and broad. We can address this using cartilage or implants along the bridge of the nose to help build up the bridge of the nose. Whatever is required, it will be done scarless and look natural, to keep your scarless secret.

Ethnic Scarless Rhinoplasty

Ethnic Scarless Rhinoplasty requires special care and attention. Often ethnic noses tend to be flat and broad along the dorsum. The cartilage is somewhat weak and flimsy and requires additional cartilage support in the form of grafts to help support the tip and build up the dorsum (profile). Sometimes nostril narrowing is necessary to achieve success and this can be accomplished with multiple techniques depending on the need.

Russian Scarless Rhinoplasty

Russia, the world’s largest country, is home to some of the world’s most beautiful faces and most talented people. We do tremendous numbers of Russian patients every year who travel to Los Angeles to have their nose done via our special scarless approach. Russian scarless rhinoplasty requires dedicated care to subtly enhance the nose without major change. Minor but meaningful refinement is the desire of most of our Russian patients. Whatever is required of your nose, it will be done scarless and look natural, to keep your scarless secret.

European Rhinoplasty

European Scarless Rhinoplasty is a unique opportunity our practice has. We see patients from London, Paris, Italy, Belgium, amongst other European cities and countries routinely. This is because of our scarless approach to the nose and minimalist mentality. Ironically some people just want less done to their nose, but most rhinoplasty surgeons inflict their “reconstructive mentality” to the nose on almost every patient. This makes it very difficult for the patient looking for a subtle finesse scarless rhinoplasty where subtle changes are most important. Whatever is required of your nose, it will be done scarless and look natural, to keep your scarless secret.

Middle Eastern Scarless Rhinoplasty

Middle Eastern noses are very special and unique. It is of utmost important to preserve the natural character of a Middle Eastern nose without changing too much. We believe in enhancing and improving the nasal contours without causing drastic change. We also believe inflicting a scar in the middle of a beautiful painting is blasphemous and recommend almost all primary cosmetic noses be done via the scarless closed method. We do noses from almost every Middle Eastern culture who fly to Los Angeles seeking subtle refinement that goes appreciated without being obvious. The stigma of a bad nose job is haunting and we do everything possible to keep our Scarless Noses as natural as possible.  Whatever is required of your nose, it will be done scarless and look natural, to keep your scarless secret.

Indian or South Asian Scarless Rhinoplasty

Being of Indian descent and training under Dr. Raj Kanodia, also of Indian descent, Dr. Dugar has a special place in his heart for Indian and South Asian (Desi) patients. Bollywood Stars to Hollywood stars choose the Scarless Nose method because of its minimalistic and scarless approach to the nose. It is of utmost important to preserve the natural character of an Indian nose without changing too much. We believe in enhancing and improving the nasal contours without causing drastic change. We also believe inflicting a scar in the middle of a beautiful painting is blasphemous and recommend almost all primary cosmetic noses be done via the scarless closed method. We do noses for Indians locally and across the world who fly to Los Angeles seeking subtle refinement that goes appreciated without being obvious. The stigma of a bad nose job is haunting especially in Indian culture, and we do everything possible to keep our Scarless Noses as natural as possible. Whatever is required of your nose, it will be done scarless and look natural, to keep your scarless secret.

Models

For Models looking for nasal refinement, there is truly no other logical choice than the Scarless Nose method. This is why virtually every model around the globe chooses Scarless Closed Rhinoplasty for their nasal refinement. “Open” rhinoplasty surgeons will tell you that 99% of their patients have no obvious scars, but if you are the 1% who gets a bad scar under your nose, it could be career ending. We opt for an even more subtle and even more natural scarless rhinoplasty for our modeling and on-air patients due to the tremendous demand to look natural. A bad nose job can destroy a career, so it is of utmost importance that a subtle, natural scarless rhinoplasty be the goal via a scarless closed technique. Often too much is done to noses with the “open” reconstructive mentality and that can be disastrous to a beautiful nose that simply requires subtle refinement of the bridge, tip, or bones. Whatever is required of your nose, it will be done scarless and look natural, to keep your scarless secret.

Actress or Actor

Whatever is required of your nose, it will be done scarless and look natural, to keep your scarless secret.

Celebrity Scarless Rhinoplasty

For prominent celebrities looking to refine and enhance their nose, great contemplation must be taken. There is truly no other logical choice than the Scarless Nose method. This is why virtually every major celebrity around the globe chooses Scarless Closed Rhinoplasty for their nasal refinement. “Open” rhinoplasty surgeons will tell you that 99% of their patients have no obvious scars, but if you are the 1% who gets a bad scar under your nose, it could be career ending. Broken Nose or Nasal Fracture

After a traumatic nasal injury, the two biggest concerns are nasal obstruction and cosmetic deformity. Please contact our office as soon as you or a loved one experiences a nasal injury. We will help guide you through this traumatic experience with our significant expertise. Dr. Dugar has repaired hundreds of nasal fractures through his career successfully allowing patients to breathe and look better after their injuries. Sometimes immediate surgery can be done to help reset the nasal fracture if it is extremely severe, but when it’s a mild deviation, sometimes waiting 6 weeks til most of the swelling has gone down is the most prudent to analyze the true outcome. Surgery for nasal fractures often includes fixing a broken nasal septum as well and can include straightening the nose as well. Whatever is required of your nose, it will be done scarless and look natural, to keep your scarless secret.

Athletes

Peak Performance Athletes routinely choose Scarless Nose surgery to address their breathing and functional needs.  Oxygen is the key ingredient in our body’s ability to function at high levels.  When you have a nasal obstruction, you are in essence decreasing the body’s oxygen levels because of the decreased nasal airflow. When it comes to athletes, we have successfully treated NBA players, NHL players, Tennis players, NFL players, Olympic Swimmers, Peak Performance Coaches, and more. The biggest key to treating athletes comes from creating a peak performance breathing enhancement. Breathing issues from the nose can come from multiple areas:

  1. Septum
  2. Inferior Turbinates
  3. Nasal Valve
  4. Allergies
  5. Polyps, Chronic Sinusitis, or other medical conditions

We identify and resolve each of these breathing functional issues causing nasal obstruction to provide a peak performance breathing state from the nasal cavity to allow increased oxygen levels and energy flow into the body.  This translates to better sleep, better oxygenation, and better athletic performance. Whatever is required of your nose, it will be done scarless and look natural, to keep your scarless secret.

The Five Things I Hate About Rhinoplasty

  1. Unnecessary external cuts and scars (99% of surgeons perform “open” rhinoplasty inflicting a scar at the base of the nose)
  2. Pinched Nasal Tip (when too much cartilage is removed and/or no support is provided causing a collapse of the tip)
  3. “Ski Slope” Profile (when too much cartilage and bone is removed from the profile causing a severe scooping of the nose)
  4. “Miss Piggy” Nose (when the tip is lifted too much)
  5. Worsened or Unimproved Breathing (when nasal breathing and structural function of the septum, turbinates and nasal vales are not adequately addressed)

These five things are constantly in my mind during every consultation and every surgery.   This is where being the only surgeon in the USA whose entire surgical practice is dedicated to Scarless Rhinoplasty allows a superior advantage of finding the perfect delicate balance of meaningful refinement to a nose without drastic uncomfortable change.  Natural noses are mandatory in my practice.

  • HONESTY IN ASSESSMENT

“Often, I will tell patients, ‘your nose looks great with your face. Don’t touch it!’ This might sound shocking from a nose surgeon, but I believe in integrity over operating. Not everyone needs surgery. And of those who do, not everyone qualifies for a scarless nose surgery (i.e. some severe revision cases or reconstructive cases). I am brutally honest with my nasal assessment, so rest assured if you are accepted as a Scarless Nose patient, it is because I am passionate and excited about the result I will deliver for you. “

  • MY ONLY ACCUSATION

“The only thing I get accused of is doing too little to a nose; and I’m totally okay with that. This accusation happens rarely but when it does, I’ll be the first to admit I’m Mr. Conservative with my scarless noses. I do noses day in and day out, so I have learned that conservatism is the safest route for natural, long lasting results that both patients and I are thrilled with for years to come.” – Dr. Deepak Dugar
